Student-Athlete Forms

The NCAA requires a comprehensive athletic pre-participation history and physical examination no more than six months prior to the first practice.  The sports physical form must be filled out completely by both the student-athlete and the doctor at the time of the visit.  Without the appropriate completed forms on file and full clearance from the College of Saint Elizabeth athletic director, the student-athlete will not be allowed to participate in practice or competition.
